Chronicles Of Nurse Atutupuyoyo....

Nurse Atutupuyoyo has come again with a story that might damage your ear drums if told to you Orally......lol





*Please read to the end and give recommendation on what a nurse should do*

This morning a woman was accompanied by her husband and neighbours on account of  her privates bleeding for about five days now....

On examination she was really pale. I queried ectopic pregnancy but after I did a thorough history I was persuaded to do a pregnancy test for her which was positive.

As I was about to tell her husband, she signalled me. I became curious and dismissed the husband to allow more privacy. She then revealed that she did medical abortion but the blood flow wouldn't just stop and the husband is not responsible for the pregnancy
Note she has 4 kids for her husband and the last is 10years old.

She wants that I do not to tell her husband.
And her husband confided in me to tell him the truth, that neighbours are suspecting that she is having a miscarriage.

*As the nurse, Please what do I do ethically.?
Do I lie for the woman to save the marriage or should I tell the husband the truth*

For now I just told the man that she might have something called endometriosis.

This is a real life issue o. It's not fictional.

Note: I had to stand in for the doctor because he had like 2 emergencies to attend to.

*Hmmm this is tough but why can you not tell the man that she is miscarrying instead of an abortion?Or has the man not slept with her in a long time to warrant that?
Why dont you hand the case over to the Doctor if you cannot decide on what to say?Please do not tell the man anything oh.

      I am a woman and a doctor.
      If this lady has her husband as "next of kin" in her medical records,
      and I sense that she has put her life in jeopardy,
      I am under obligation to tell the husband what happened.
      If not, it is the same husband that will turn and accuse the hospital of killing his wife
      like we read in this blog about the expatriate husband and his dead Nigerian pregnant wife.

      When women of easy virtue useless their lives and fertility in the hands of quacks they come to some innocent doctor's hospital to die and you will tell the medical personnel to keep quiet and absorb the guilt and liabilities?

If you know your ethics and privacy rule, HIPPA you will know that as long as the woman is above 18, mentally capable and does not have a case that's needs to be reported to department of health, you are held by the principle of privacy and confidentiality. If you do otherwise that woman might team with the husband and she your hospital for millions. Check you board of Nursing. Leave sentiments out of this.

    4. Don't tell the man, I believe there is something called doctor(nurse) patient confidentiality. Thats why you are a professional, she is your client, you are obliged to keep her secret. Let he carry her burden, but make sure you involve the head doctor so you won't be undermining he/she authority.

      But like the case @14:08 mentioned which I read here, the woman slipped into coma
      The medical professional has to walk the tight rope of "confidentiality/well being"
      while avoiding litigation in the process.
      Assuming that lady in this chronicle dies, and autopsy reveals that she has "retained products of abortion"
      (which is the likely cause of the bleeding), won't the husband sue the hospital for providing him with false information
      or probably for "negligence?"
      As you can see, the nurse hasn't told us that they did any "evacuation"
      Again, the problem is likely to continue and the husband takes the wife to another hospital and what they find out is different from "endometriosis"
      is that not a ground for litigation?
      Instead of the nurse to provide a false diagnosis, she should have kept mum and told the husband that the wife will explain
      in details to him.

    6. Madam nurse, who is the patient? The woman right? You have no business telling her husband. Whatever happened to Dr-Patient confidentiality? I know you are not the Dr but it still applies.
      I'm not saying the woman is right but it's not in your place to tell the husband. They should sort out themselves outside of the hospital.

  2. I'd advise you consult the doctor first, and tell him the truth and the situation on ground should the husband try to contact the doctor later to know if you lied to him or not, On the doctors advice you can then proceed on what to do.

      Tell her husband that she had a miscarriage. It is left for her to tell the husband who is responsible for the pregnancy.

    Tell the man his wife is in a better position to tell him but you will ensure the bleeding is taken care of.
    It is not in your place to cover up her atrocities.

  9. I had similar cases, a patient whose Hubby just came back from a trip and the wife probably got preggy for someone else, she had an abortion which was complete, but came in weak and pale. She was being treated and told me to please not tell her hubby what her diagnosis is, he suspected, of cos anyone he met told him something vague as we didn't want to lie outrightly, but we couldn't say the diagnosis, patient doctor confidentiality , had another preggy with HIV! She kept trying to infect her hubby, while hiding to take her ARVs, and warned that no one should reveal her status. Crazy world

  12. She is your patient so you have to maintain patient-doctor/nurse confidentiality as there's no legal obligation to inform her husband. Don't be deceptive tho, lying is protecting her! However, you can ask to discuss her condition with her husband if it's serious and let them figure it out for themselves. Mahn! It's not your place to protect her

    Tell the truth. Let the wife deal with the consequences. It's a no brainer.
    She could get complications later and die and it is your report that will nail you.

Dear Nurse, pls refer to your ethics book and know what you are expected to do.but in my field as a medical doctor,I am obliged to get the wife's consent before I disclose her diagnosis,but if she were in a position where she cannot give me consent e.g. unconscious,or in an emergency situation where her life is at stake and her husband is the next of kin,then I can tell him.so for now convince your patient to tell her husband herself and excuse yourself from this issue.

  21. Possibly the man isn’t doing the woman. Reminds me of a similar case , where the woman had a prolapsed uterus, she was booked for repair as part of pre op investigations was done alas she was preggie, now hubby had not been servicing her cos of the condition. However in this case she probably didn’t know and the doctor didn’t know hubby wasn’t gbenshing her na so he share result . In short the marriage just ended right in that consulting room
