The report, issued on Monday, concluded that the crash was primarily caused by the pilot’s decision to continue flying under visual flight rules despite entering instrument meteorological conditions.
According to the report, this critical error led to the pilot’s spatial disorientation and subsequent loss of control of the aircraft.
In addition to pilot error, the NTSB cited systemic failures by the helicopter operator.
The agency said the company’s inadequate oversight of its safety management processes contributed to the crash. Investigators found lapses in pre-flight risk assessments, maintenance documentation, and regulatory compliance.

ReplyDeleteThis was what the owners of ADC Airline then, especially Captain Okon, used to do; and wasted lives. May their souls continue to rest in peace with God. Amen
DeleteKai....avoidable death

May their gentle souls rest in peace
ReplyDeletePilot is the designated fall guy. Probably , he didn’t take the decision to fly alone…
ReplyDeleteThe whole police helicopter fleets of the area was put out of operation for that day because of the weather. Maybe money was the basis for that operator to agree to fly that mission. They should be charged huge compensation for such a tragic oversight.
ReplyDeleteThere is always someone willing to take a risk no matter the situation. I cannot believe that these ppl would be alive today if safety measures were adhered to. And to think that they were not even flying for a life and death reason (no pun), they simply were heading to the Superbowl, something that would only be annoying if missed, nothing else.
DeleteGod knows best
