MAGODO SPECIALIST HOSTPITAL ON THE DEATH OF MRS CHISOM ANEKWE.

We are deeply saddened by the death of Mrs Chisom Anekwe. We wish to express our sympathy to the family and friends on this unfortunate and tragic incidence. We pray that God will comfort them and grant them the fortitude to bear this great loss. We are however compelled to address the postings on the social media which are untrue.

The facts have been markedly falsified and distorted. It would appear that deliberate attempts were made to damage the image of the hospital and the integrity of the attending doctors and nurses based on false premises. It has become necessary to state the hospital’s account within the limits of professional ethics in order to give members of the public a balanced perspective. We wish to state that the case is presently under investigation by statutory bodies such as the Nigeria Medical Association (NMA), the Medical and Dental Council of Nigeria (MDCN) and the Lagos State Health Facilities Accreditation and Monitoring Authority (HEFAMA) and as such, the hospital would not want to do anything that would prejudice such investigations. It is also hoped that a post mortem examination would help to unravel the cause of this unfortunate incident.

Mrs Chisom Anekwe was a pleasant and gracious lady who was well liked by staff and other patients. We were delighted when she registered for antenatal care in Magodo Specialist Hospital for her third baby. She had had 2 previous successful deliveries in this hospital in 2012 and 2015 respectively. During the index pregnancy, she developed hypertension at a time when the baby was not yet matured for delivery. Hypertensive diseases (pre-eclampsia) are the leading cause of death associated with pregnancy worldwide.

She was offered admission to control and monitor the blood pressure on 26 April 2017 but declined and went home. She later returned on 27 April 2017 because of headache and inability to sleep. She was placed on BP lowering drugs and was regularly seen by doctors and nurses including gynaecologists. At least 3 gynaecologists attended to her individually or jointly during her admission.

She remained stable until the early hours of 30 April 2017 when she complained of sudden severe abdominal pain. It emerged that she did not take the medication prescribed to reduce her blood pressure the previous evening. The nurses and doctors including a gynaecologist came to see and made a diagnosis of placental abruption, a condition in which the placenta is detached from the womb before the onset of labour. Placental abruption is a known complication of hypertension in pregnancy and in this condition, the patient would lose a lot of blood and the baby would suffer from nutrient and oxygen lack and is usually born dead. On assessment, the patient was in shock.

Immediate steps were taken to resuscitate her with intravenous drips and oxygen while attempts were made to obtain blood for transfusion. The staff then prepared for emergency caesarean section to try and save the baby. The team including gynaecologist, anaesthetist and paediatrician were called and they attended. But before starting the operation, the doctors needed to confirm that the baby was still alive by listening to the heartbeat. Unfortunately, the baby’s heartbeat was not heard.

This necessitated a change of plan. They decided not to proceed with the operation because the baby was already dead and the operation could be dangerous for the mother in that situation. A further complication of placental abruption is disseminated intravascular coagulopathy (DIC) a condition where the blood fails to clot. This could lead to excessive and uncontrollable bleeding during a caesarean operation. This was explained to the husband who gave consent for labour to be induced. Labour was induced and she was monitored throughout. She then had a normal delivery of the baby and placenta following labour induction.

Resuscitation of the patient continued and she was given drips and blood transfusion to replace what she had lost. She was continually monitored by nurses, doctors and gynaecologists. She was never left on her own contrary to the postings on social media. There was initial improvement in her condition, which appeared to be stabilizing. But when it unexpectedly started to deteriorate, it was decided after consultation with the husband to transfer her to another hospital.

The story on social media suggested that the patient fell into labour, called for help and nobody came. This is not true. The claim that the patient fell into labour was contradicted by the same social media post which alleged that labour was induced without the husband’s consent. The patient never fell into labour and when she developed placenta abruption, the doctors and nurses including the gynaecologist attended promptly. For most of the time, 2 gynaecologists jointly attended to her along with other doctors and nurses. The patient was monitored continually during the management of this severe emergency.

The decision not to proceed with caesarean section was taken in the patient’s overall best interest and according to standard medical practice. The theatre was prepared and the team of gynaecologist, anaesthetist, paediatrician and others assembled ready to do the operation. They did not proceed because of consideration of the patient’s overall best interest. Ironically, it would be financially more rewarding for the hospital to do caesarean section rather than offer vaginal delivery. We placed our patient’s interest above any potential financial gain.

The story also claimed that the “doctor sneaked out of the hospital”. This is untrue. Two gynaecologists were jointly attending to the patient and when her condition appeared to be stabilizing, one of them left with a plan to return after a short while. And when he was contacted that the patient’s condition was deteriorating, he immediately made his way back to the hospital and later proceeded to the hospital where the patient was referred to support the family. He left the referral hospital after he was physically assaulted by the patient’s husband.

We care very passionately about the health and lives of our patients. The hospital is well equipped and adequately staffed. The hospital is fully accredited by the relevant statutory authorities. We are very sorrowful about losing this patient. The death of any young person is regrettable and must be guarded against. We have handled many difficult cases successfully in the past and did our best in this situation. But we also are humble to recognize that doctors and nurses may care, it is God that heals and that despite human best effort, death may still occur. We are greatly distressed and saddened by the death of this young woman who had been our patient for over 5 years and had her 2 babies successfully under our care. Our hearts and prayers are with the family. We pray that God in His infinite mercy will comfort them and grant them the grace and strength to bear this great loss. We pray that the almighty God will take care of the children and the family she has left behind.

    1. Please...... How can a patient on admission not take her drugs?... What was the essence of the admission in the first instance!!!! Was she not supposed to be under constant check/observation???????????

    2. Hmmm....
I had and lost a baby last year after a supposedly uneventful pregnancy... Believe me when I say safe delivery isn't dependent on any medical or experienced hands. Only God guarantees that
So many questions I asked and majority of the the answer is "Nature"..pls who or what is nature?
Why will induction fail one woman and help another?
What makes a previously, young and active lady suddenly develope preempclasia?
Why do some rural women birth their babies healthy even without Folic acid yet a modern still gets a baby with neural birth defects?
Why do some pregnancy past term while some just fall into labor at 37 weeks exact?
      All answers is "Nature"...
      Only what God permits happens.
I pray her husband finds solace and her kids gets care and love.

    3. So sorry dear for your loss.
A lot goes on with the body during pregnancy and I think people react differently. Some women develop blood pressure due to stress, anxiety etc.Hmmm God knows best!!

    4. Sorry for your loss Anonymous 17:10, a friend of mine went through same 2 weeks ago and lost the baby boy she's been hoping and praying for. This happened in the UK.
Child delivery is all in Gods hands and that's why Yoruba's greet the new mum or family "E ku ewu omo" as you never know what will happen during pregnancy or delivery.
May the lady's soul rest in peace and God be and keep her children

That's how the hospital where the girl with Oyibo Old cargo husband told their version and threw us into confusion.

If you ask me, even on facebook a random medical personnel faulted the deceased hubby's story.

So I kinda believe this hospital.

Women go through so much during pregnancy that it is disheartening to die in the process.

  5. I believe them.
People tend to get irrational when they lose a loved one, hence the husband's behaviour.
She's had two babies with them, why would they do all that they were accused of to her?
May her soul rest in peace.

    2. But wait they say it emerged that she didn't take her medication. You're telling me your hospital left it in the hands if a patient on admission in such a state to take her meds. You didn't have a nurse take it to her, watch her take it and record she had used it. Nonsense

      Delete
    3. Yes sunshine, people act irrationally at times but there are holes in this story
1, A patient on admission didn't take her drugs, really? What are the nurses there for?
2, The doctor did leave the hospital while the patient was in pain so the husband wasn't lying about that. He couldn't handle it so he raced hence the reason why he wasn't there for the transfer of patient. What exactly did he go to the next hospital later to do?
Did they have to add that the bereaved man assaulted him? More like they are trying to discredit the witness to make us question his sanity...
      Learnt that from HTGAWM
RIP beautiful, I hope you get justice!

    4. Yes it's possible for you o admission to not take your drugs. I was on admission for about 7 weeks so years ago and I did not take some drugs given to me. I will pretend to put them in my mouth but I still had them in my hands and then drink water. It was the cleaners that saw my drugs under the bed that busted me. So it's very possible a patient on admission will dodge taking their drugs

      Delete
    5. Yes it's very possible to be on admission and not take your mess. Some patient refuse to take their mess. You can't force them you just have to reassure and counsel them, it will be documented that patient refused meds

      Delete
    6. The nurse can only give you ur drugs but will never force a patient to take his or her drugs some will pretend they take the drugs in ur presence while they didn't

  6. Hmmmm....let's help the health professionals help us. Pls take your drugs when asked to especially when you are in a dire situation. Fine and young woman, gone like that.

My choir coord's wife who had bp too delivered her baby a month before the due date. I remember once where she was injected after she had started walking round and she became so dizzy. Even once I was told to ensure she took her drugs immediately, but after trying to make her wake and she still was drowsy, I put the drug in her hand and guided it to her mouth and spooned water in.
With what I saw from those that have c.s, I raise my hands for those omugwo stories where some women had no one to help them.

  7. The thing wrong I see here is thhat she should have been sectioned Long ago, once d BP was noticed to be uncontrollable. Instead of waiting to "gain more dayss" for d baby's development. It's just a matter of placing d mother above d baby. It's d same thing that killed Moji in the abroad there in Canada, so u can imagine the level of emergency we are talking about here. Pregnancy is not a joking sturvs oo. I don't know how our ancestors were surviving it oo. God's grace I guess. If u born finish, kneel down and thank God, cos it's not a small something.

    3. You don't just jump on the c.section wagon. If the baby isn't mature enough you wait for the baby to be old enough i.e wait for the baby to be viable and then give betamethasone to the mum so the baby's lungs will mature. Pre eclampsia is not an indication for a c.section all you can do is blood pressure control. I'd like to believe that the Dr in front of your name isn't from being trained as a medical doctor.

    4. Are you sure you are a medical Dr, so if you client has pre eclamsia u go into the theatre for cs hian na wa for u, you won't monitor the bp & do something to lower it. so u section a patient with HPB wonder the kind of medical personnel you are.

I had and lost my baby in April too. I used a general hospital for my antenatal.
I started behaving hbp in my seventh month. I asked if there was any medication to bring down my BP but the answer was always " madam just have enough rest".

After all said and done, no drug was prescribed for me. Though I read about adolmet(not sure of the spelling) on line. I didn't take it since it was not prescribed and I didn't want to self medicate.

At 37 weeks, I was told to do a scan. The result was heart wrenching.

Please all this talk about general hospitals been the best is not true.

They may have equipments but their lackadaisical attitude has resulted in so many deaths. Na human being go use equipment Na.
Then different gynaes will attend too you at different times.

All through my ANC, I newer saw the same gynae. You see A today,Dr B next week and it continues like that.
You ask questions and they just ignore you.

So many things to talk about.
      I thank God am alive.

The God that give me the first will not let affliction arise the second time.

      I have hope...

I remember when I was at a private hospital in Surulere, the nurses were engaged in gossip and the woman, obviously close to being in labour walked all the way from her room to scream at them, saying if she dies that she'll make sure no child survives them. I couldn't help but laugh that day. Sometimes, one has to be stern with these people. They are very mean.

    3. God bless you for this comment joor.I know things can go wrong in certain situations but let these professionals take the lives of their patients more seriously.

I had surgery in the UK and my 1st and 2nd night after the surgery was hell. You need to see how they were checking me every 10mins, disrupting my sleep, calling my name, checking blood pressure and pulse etc. At a point I was tired of life that these people should let me sleep, I just had surgery. But they knew what they were doing.

It got better 3rd third night and the nurses were coming every 45mins, very hardworking nurses, I don't know how they do it.

Some of these useless Nigerian nurses are witches who fly at night, so nasty to pregnant women. You hear them saying things like when you were doing it, was I there? Don't bother me and all sort of bulshit that you dare not utter in sane climes.

  23. Pre eclamsia is very deadly. I had my son 2months early. And I must say that it was the doctors right thinking that saved my life. The doctors are clearly at fault here. They placed the life of the baby above the mother. Women with this condition must be delivered through cs. The labour is too hard on their blood pressure and will most likely lead to convulsive fits and death. Please admit your error judgement and apologise to the family.how can the baby die and you people didn't even know. That means the bp was uncontrollably high and the baby had not been getting oxygen for at least 24 hours which would cause amniotic fluid to dry up. And you people did not notice this on time? You did not make the right call to evacuate the baby on time wether dead or alive to save the mother.

    1. Preeclampsia is not an indication for a c.section . The only definitive cure is delivery and removal of all fetal placental tissue but you can defer delivery in mild preeclampsia. So you manage it based on gestational age. If it's before 36 weeks as long as the mother and baby are stable you do what we call watchful waiting that's basically watching to see if it progresses to severe preeclampsia if the gestational age is more than 36 weeks you induce labor with IV oxytocin and continuously infuse IV MgSO4 to prevent seizures. Only in severe preeclampsia do you deliver immediately by IV labetalol to reduce Bp, IV MgSO4 to prevent progress to eclampsia and oxytocin to induce labor. The only time you do a C. Section is when there are obstetric indications.

    4. @anonymous 19:12. You are so right in your analysis for preeclampia. I had high BP in pregnancy/malaria and all my private hospital doctor could give me was diazipam even when my BP kept going up on each ante-natal appointment, give me BP drugs he said no need. Thank God I traveled. Gave birth with BP of 210/174. Labetalol and later Magnisum was given when labetalol wasn't working fast enough and an hypertension specialist had to be called in. Placenta had already detached from the womb and baby had to be . Will send my labour story when am up to it. If I gave birth in that hospital, am sure it's press release they wld be giving. Ordinary malaria injection that I was given burnt my bum and almost turned to an abscess.
Some of these doctors need to go for training every now and then.

  29. At seven months pregnant with my first,I was seriously sick and vomiting blood.i was rushed to UPTH where I registered. On getting there, the nurse I met at the antenatal ward asked what's wrong. My hubby tried explaining, she just shushed him up and said can't she speak for herself. I managed to explain myself..next thing she said was "when is your clinic day"?I told her Thursday (which was the next day). She got so angry and said "why didn't you wait till your clinic day before coming". Before I could open my mouth to reply her,my hubby just jejely wheeled me out of the hospital and straight to a private hospital.I was attended to in less than 15 minutes. Turned out I had a terrible ulcer. Only God saw me through that pregnancy.

Probably why they dilly dallied and eventually induced her.

How can they even say she didn't take her drugs? Was it not their responsibility to give her drugs? Or did she tell them that she hid it under her tongue?

Make we take am easy with the bad words about our hospitals cos I knw hospital death occur everywhere in the world, late keffe was pregnant & she died in the US, even late moji died of pregnancy complications yet no one accuse the system of killing her, but here even with pple that refuse to take their drugs, refuse to adhere to medical advice but their pastors advise turn to blame Dr & nurses when their case gets messy, we should change our Itk attitude esp with our health & allow the health professionals to do their jobs & support them, Pls let's remember that they are not magicians, No Dr or Nurse will leave his house just to the hospital to kill the patients but most of us esp the financial buoyant, ITK groups tend to know more than everybody & refuse to take simple instructions even when their live is at stake. Rip to the late chisom is a pity she died Pls we all should learn something from this write up, take ur drugs as @ when due, ask questions politely, adhere to medical advice is just for ur own good. Selah to everyone.

  33. I had eclampsia due to blood pressure. I was on admission in an American hospital, was taking my medication and still had a seizure. I was very lucky my husband was in the room when it happened so I got help immediately. I was unconscious when they had a c section to remove my baby. To the glory of God we are both alive and he is 11. What I don't understand is why they left the baby that long. A woman that stressed didn't need a normal delivery.

  35. Hello everyone,

This is my first time of commenting on this blog although i have been visiting since 2015. i am forced to share my experience because i would have been history today but God still have plans for me and that is why i can type today. I was a victim of pre eclampsia as well and i used PHC badore for my ANC, when i was about 37weeks i was diagnosed with HBP and all they did was to ask me to start my leave immediately without any medication,i proceeded to ibadan to birth the baby in a private hospital and after seeing the gynecologist she gave me some medications and asked me to go back home and come back in a week time. At exactly one week i went to the hospital and convulsed at the reception i woke up in the theater room and i felt every cut. my baby died and my world crumbled because i knew the stress i went through before getting pregnant. Reading about this story makes me sad but at the same time thank God for the grace bestowed upon me. I felt funny last week and did a pregnancy test and it was positive. I would like to ask the doctors in house if its not dangerous because i had that baby 23rd January 2017(through CS). i have not told my husband or anybody because i want to be sure my health is not at risk before making the announcement.
