  7. To the Anon who asked if it can happen in developed countries. Yes and it happens. It may be DVT/PE. The risk factors apart from diet, weight, lifestyle are genetic issues relating to factor V Leiden (FVL), factor v111 levels, sitting in traffic without stretching your legs or walking, being on one spot for hours, essentially any sedentary lifestyle can be a risk factor for Deep Vein Thrombosis and Pulmonary embolism.

Asking the maid to do all in Naija doesn’t help your health walk 15 miles between going to work and doing my housework with no maid in a 3 floor home. Not having a maid means we climb the stairs and walk around a lot even at home. Before even stepping on a treadmill or jogging. Telling your maid to do everything is not good for you. You need to walk around after time in traffic. There was a a story about a patient who returned from a trip to Nigeria (14 hours plus waiting time) and reported for swollen leg days after. The urgent care clinic did not ask the right questions as that would have made for a more accurate diagnosis but just gave him a muscle relaxant. By the time he returned to the ER the next morning he lived for less than an hour. It’s very easy to miss. It can present as innocently as pain in the calf or a swelling on the ankle or just leg pain or even shortness of breath

If you sit on a plane or in traffic for hours and your maid helps you run around the house while you sit and eat mixed meat and any type of food, you are at risk. I asked a couple of Naija physicians here who didn’t seem to grasp the severity and potential fatality. The right diagnostic test initially is a d-Dimer test and ultrasound and the definitive test is a CT scan but there is very little time by the time folks present in the ER. That is a reason I always hit on our millionaires and billionaires to copy their mates here and come together to build, equip and train personnel for a world class hospital in Nigeria. Hospitals are not owned by the government in the US. The only exceptions are the county or local government clinics and the Veterans hospital system for soldiers. The hospitals here are owned by charities, private donors with millionaires and regular donor setting aside money for the poor. A reason nobody get turned away no matter how severe their case is and no matter their nationality. It’s also the reason you are not asked to deposit a huge sum. The rich give back not with rolls royces and ferraris for family but by donating huge sums to research and treatment of diseases and helping the poor.

Not all diseases can wait 6 hours till you get to London or France or Germany, the reason ambulances use sirens and folks stop to make way for them knowing someone is in great need of quick care in the ambulance.

it was the reason COVID19 was and it still is deadly as it causes clots and PE so they had to go on ventilators initially but now they add anticoagulants to dissolve the clot the virus causes in the lungs as part of the treatment protocol so no one is talking about ventilator shortage again as newer more effective anticoagulants are available to add to the protocol. Pneumonia was initially thought to be the reason for the lung issue requiring ventilators. Thank God knowledge is increasing about the terrible disease. We now know it wasn’t bacterial pneumonia superinfection but pulmonary emboli or PE that was killing Covid patients very quickly.Avoid self medication at all costs and use the more experienced physicians that went to the more resourced universities to train and who still stay abreast of modern research. May God help us all.
